* EXTRA-HELP DEPUTY LDF PLAN-IV - NEW ENROLLMENT PAYMENTS *
Please read the below carefully and select the appropriate payment choice to finalize your LDF Plan-IV enrollment.
In order to obtain extra-help LDF coverage, you must also be a Retired Associate Member of PORAC (RAM). If you aren't already enrolled in PORAC RAM, we will take care of that for you. The fees collected here to enroll in you PORAC and LDF are as follows:
- Annual PORAC RAM Membership fee (Paid for by the DSA if you are a retired DSA member): $48.00
- Annual LDF Plan IV fee: $96.00
- Paypal processing fee: $6.00
TOTAL COST FOR RETIRED ACTIVE DSA MEMBERS: $100.00 / Yr.
TOTAL COST FOR ALL OTHERS: $150.00 / Yr.
If you interrupt the payment process, your renewal will fail and you will need to start over.
We have done our best to account for new Extra-Help members who join us mid-year. In order to not overcharge you, please select the quarter in which you are enrolling in PORAC and LDF. We do our best to only bill you for the months you will actually be covered. After you initially enroll through us, you will become part of our annual December enrollment group to pay once per year for a full year. We will notify you along with all other eligible members we it's time to renew your coverage each December.
